Wisconsin: medical director and delegated services for an addiction medicine or MAT clinic

Physician licensed under Wis. Stat. ch. 448 (medicine and surgery, including osteopathy); Wis. Admin. Code DHS 75.59 further requires at least 1 year of experience in addiction medicine or addiction psychiatry (board certification preferred, or working toward it, per DHS 75.03's medical-director definition) and, if that experience requirement cannot be met despite documented recruitment efforts, a specific plan to acquire equivalent training/skills within 4 months of hire.

• Overseeing all medical services performed by the opioid treatment program
• Ensuring the program's compliance with all applicable federal, state, and local statutes/rules governing medical treatment of opioid use disorder
• Reviewing and countersigning patient treatment plans on a quarterly basis
• Approving take-home medication privileges and changes in take-home dosing frequency
• Maintaining valid DEA registration and any required buprenorphine/methadone waiver documentation
• Documenting evidence of physiological/psychological dependence, addiction history, and any state-opioid-treatment-authority (SOTA)-granted admission exceptions in the patient's case record before the initial dose is administered

The program sponsor need not be a physician, but the service must separately employ a licensed physician as medical director (DHS 75.59), distinct from the federal 42 C.F.R. Part 8 baseline.
